Continuum Health Alliance Selects Caradigm For Population Health

Caradigm, the leader in enterprise population health, recently announced that Continuum Health Alliance, a physician enablement company providing practice management, population health and community care services to provider groups, has selected Caradigm as part of its population health strategy.

Continuum provides a next-generation health platform, enabling independent physician organizations, hospital-based physician groups, commercial accountable care organizations (ACOs) and MSSP ACOs to achieve the Triple Aim; improve quality of care; enhance the patient experience; and lower the overall cost of care. Continuum will add Caradigm to its integrated health platform, combining Continuum’s existing best-of-breed technologies, care management best practices and expertise with Caradigm’s population health solutions to scale its managed care services business as it assumes an increasing number of value-based contracts.
